Study Of ESR, CRP And WBC Levels As A Marker In Immediate Post-Operative Orthopaedic Infection

Abstract
Aim - Our study is to evaluate the levels of Erythrocyte sedimentation rate, C – reactive protein and WBC as a marker of immediate post-operative orthopaedic infections. Basis of the study is to evaluate the value of Erythrocyte sedimentation rate, C – reactive protein and Whole blood cell on day 0, day 3 and day 7 of surgery as markers of infection. The physiological response in infected cases was measured by mean values of Erythrocyte sedimentation rate, C – reactive protein and Whole blood cell plotted on a bar chart and compared with that of the non-infected cases. Methods and material - We studied the kinetics of Erythrocyte sedimentation rate, C – reactive protein and Whole blood cell levels in 200 post-operative orthopedic cases. Out of 200 cases operated, 193 were found uneventful and remaining 7 presented with signs of infection in form as fever, local rise of temperature and serous discharge at the end of day 7. Result - There was no significant difference in the Erythrocyte sedimentation rate, C – reactive protein and Whole blood cell level between the infected and non-infected cases on DAY 0. On DAY 3 there was significant difference in the levels of C – reactive protein in infected[0.015(S)] and non-infected cases. Also, significant difference in ESR[0.001 (S)] amongst non-infected cases was noted. On DAY 7 there was significant difference in the levels of C – reactive protein in infected and non-infected cases. Conclusion - Our study does not show Erythrocyte sedimentation rate, C – reactive protein and Whole blood cell as a definitive marker of infection in immediate post-operative cases. However, a rising trend must be observed to suspect infection.
