Tension Pneumothorax during Liver and Kidney Transplantation in a Patient with Adult Polycystic Kidney Disease
Journal Title: Journal of Anesthesia and Surgery - Year 2017, Vol 4, Issue 1
Abstract
Administering anesthesia to patients undergoing liver transplantation often yields intraoperative hemodynamic instability During this type of major surgery many factors may contribute to hemodynamic collapse including myocardial infarction pulmonary embolus bleeding and preload reduction owing to inferior vena cava clamping Pneumothorax a potentially dangerous condition can arise unexpectedly during anesthesia Diagnosis is based on exclusion as the initial vital sign changes including cardiorespiratory decompensation are non-specific Here we describe the case of a patient with adult polycystic kidney disease who developed intraoperative cardiovascular collapse and hypoxemia owing to tension pneumothorax during liver and kidney transplantation We highlight the etiology of intraoperative tension pneumothorax the sequence of hemodynamic changes and the importance of early diagnosis and treatment

Routine Mask Induction and Intubation without Paralysis may be Safer
Meticulous precautions cannot eliminate disasters with presently prevailing intravenous induction techniques. Elective mask induction and intubation with Sevoflurane optimizes safety by incorporating pre-oxygenation and...
Ultrasound Guided Popliteal Nerve Block for Operative Ankle Instability Procedures
At our institution, OrthoIndy Hospital, prior to regional anesthesia, patients undergoing operative ankle instability procedures required overnight admissions to the hospital for pain control. Techniques of regional anes...
Changing Trends in Dental Procedural Sedation
Although most dental procedures are amenable under local anesthesia but many a time depending on the extent of procedure anxiety or phobia patient needs some degrees of sedation Procedural sedation outside the opera...
Changes of Hemodynamics and Blood Glucose during Tracheal Intubation in the Geriatrics with Different Glycosylated Hemoglobin Levels
Background: The severe cardiovascular response following tracheal intubation has threatened geriatrics’ safety. HbA1c may be an independent risk predictor for the outcome of cardiovascular disease and/or DM. We investiga...
Multimodal Analgesia in Inguinal Hernia Repair Using a Cyclooxygenase-2-Specific Inhibitor: A Randomised Controlled Trial
Aim: To determine whether preoperative use of the highly cyclooxygenase-2-specific inhibitor rofecoxib combined with spinal anesthesia offers superior postoperative analgesia compared to spinal anesthesia alone. Methods:...
