The efficacy of orally fluconazole and topically 1% clotrimazole cream and shampoo containing 2% ketoconazole in the treatment of moderate to severe types of seborrheic dermatitis

Abstract

Introduction: The seborrheic dermatitis is one of the most common skin disorders (1-5% of the population), that concerns as well infants and children as adults. The role of yeast-like fungi, Malassezia sp. (mainly M. globosa, M. restricta and M. sympodialis), especially in adults, is under-lined in the etiopathogenesis of seborrheic dermatitis. The advantageous therapeutical effect of anti-fungal drugs, both topical and systemic which led the decrease in number of yeast-like fungi in the affected skin has been observed in recent years. Aim: The aim of the trial was the comparative estimation of effectiveness and tolerance of the use of systemic fluconazole (Polfarmex Firm) combined fluconazole (according to own scheme); systemic combined with topically used 1% clotrimazol cream (Clotrimazolum Hascolek Firm) and topical treatment of 1% clotrimazol cream alone in the treatment of patients suffering from seborrheic dermatitis. All the patients were treated additionally with shampoo of 2% ketoconazole (Noell Polfarmex Firm) used topically on scalp and affected skin. Material and methods: The 90 patients with seborrheic dermatitis of moderate to severe degree took part in the trial. The patients, after one month of wash-out period, were randomly qualified to one of three tested groups accordingly to used treatment: I group - 30 patients who were treated with systemic fluconazole according to the scheme: 3 capsules per week (1 capsule every 12 hours) - 6 weeks together with 1% clotrimazole cream twice a days on affected skin+2% ketoconazole shampoo. II group - 30 patients, treated with fluconazole as above in group one and 2% Ketoconazole shampoo. III group - 30 patients who were treated topically with 1% clotrimazol cream twice daily and 2% ketoconazole shampoo. The patients were observed for 10 weeks. The degree of the intensification of seborrheic dermatitis was estimated according to Scaparro et al. scale (2001) in own modification.The results were worked out statistically with the use of t-Student test and U-Mann Whitney'stest. Results: All the patients completed a ten-week - cycle of the treatment.
