The relationship of serum inflammatory markers with metabolic disorders at the non-alcoholic fatty liver disease

Abstract
Objective — to study the serum levels of proinflammatory markers and determine their role in the formation of metabolic disorders in patients with non-alcoholic fatty liver disease (NAFLD). Materials and methods. Examinations involved 92 patients with NAFLD, 48 of them were diagnosed with non-alcoholic steatosis (NAS) and 44 patients with non-alcoholic steatohepatitis (NASH). The mean age of the examined patients was NAFLD was (58.44 ± 11.8) years; the gender distribution was reciprocal. The control group consisted of 30 practically healthy persons. The parameters of objective and instrumental examinations were determined in all patients in accordance with the current order of the Ministry of Health of Ukraine No. 826 dated November 6, 2014. In addition to the standard survey, the OMRON BF-511 electronic device was monitored by the patient’s body composition (Japan, 2011). For the determination of visceral fat tissue dysfunction (VFT), the index of visceral obesity (IVO) was calculated by the M. C. Amato method (2010). Determination of levels of C-reactive protein (CRP) and tumour necrosis factor (FNP)-alpha in serum was performed by the immune enzyme method. Results. CRP was significantly increased in the group of patients with NASH compared to the controls (p < 0.05), while significant differences in patients in the NAS group were not detected. Determination of the concentration of TNF-alpha in the blood serum of the examined patients showed a significant increase in both the NAS group and the NASH group compared with the control group (p < 0.05). There was a direct correlation between the level of TNF-alpha and the activity of cytolytic enzymes (ALT and gamma-GGT), triglyceride levels, as well as insulin on an empty stomach and the HOMA-index. The dependence of ALT activity on the level of CRP (r = 0.35; p = 0.032) was established. It was established that as the activity of VFT increased, an increase in serum concentrations of both TNF and CRP was observed. Conclusions. The most significant increase in CRP and TNF-α and VFT rates was observed in patients with NASH. An elevated level of TNF-alpha promoted the development of inflammation in the liver in patients with signs of a metabolic syndrome and can be considered as a risk factor for the development of NASH in patients with NAFLD.
