The structure of motivation for self-control in adolescents afflicted with type 1 diabetes mellitus

Abstract

Objective — to study the structure of motivation for self-control in adolescents with type 1 diabetes mellitus (DM). Materials and methods. In the course of research we have examined 60 adolescents aged 12–18 years with type 1 DM, receiving inpatient care in endocrinology department of the State institution «Institute for Children and Adolescents Health Care of the National Academy of Medical Sciences of Ukraine». These adolescents included 32 girls and 28 boys. Average duration of the disease was 6.9 years. The subjects were divided into three subgroups: 28.3 % of patients had optimal glycemic control, 30.0 % had suboptimal glycemic control and 41.7 % had glycemic control with high risk of decompensation. In order to study and evaluate the motivation for self-control we have used «Questionnaire for studying afflicted adolescents’ motivation to treat diabetes mellitus», developed by the author. Results and discussion. The results of our own studies prove the impact that the motivation level has on the effectiveness of maintaining blood sugar level. We have identified directives for treating type 1 DM, characteristic to afflicted adolescents with various levels of glycemic control. It has been determined that characteristic for adolescents with optimal level of glycemic control is motivation oriented towards the long-term future. Adolescents with suboptimal glycemic control lean more towards the short-term future, which enables them to spend time with friends. Adolescents with high risk of DM complications tend to shift responsibility for control of their own affliction to doctors. Almost in half of these adolescents we have identified a desire to receive secondary gain from the affliction and the lack of insight as to their own future. Conclusions. Application of the proposed method of evaluating motivation for self-control of type 1 DM enables self-control school specialists to promptly and in a quality manner, using visual scale profile of patient’s motivation, determine the direction of correctional activities during the organization of medical psychological assistance for this group of afflicted adolescents.
