Abstract
In the year 1910, Demetrius Chilaiditi described asymptomatic cases that looked like locations of intraabdominal free air at the routine abdominal or chest X-ray examination, which were caused by interposition of the colon between liver and right hemidiaphragm. The clinical symptoms are not obligatory associated with Chilaiditi symptom. This syndrome is characterized by symptoms caused by unusual anatomical location of the intestine: abdominal pain, sensation of stretching, bloating, nausea, vomiting, flatulence, defecation, as well as rarer manifestations such as substernal pain, cardiac arrhythmias, dyspnoea and respiratory disorders. In males, this syndrome is revealed fourfold more often, than in females. Chilaiditi symptomwas detected in patients aged 5 months to 81 years, but mostly often (1 %) in the population of elderly people. The authors described a clinical case of the visualized hyperpneumatosis of the colon with clinical manifestations, caused by right-sided subdiaphragmatic Chilaiditi syndrome and left-sided subdiaphragmatic dolichosigma, observed at Кyiv city clinical hospital № 8. Male patient of 80 years complained on constipation (which he had long period of time), sense of distention, abdominal flatulence with discharge after his estimation of big amount of gas especially during shaking transportation. Moreover, the patient periodically noted pain in left subcostal space, sometimes it was epigastric, sometimes it was accompanied with heartburn, removed with antacids; nausea with vomiting was rare. In the year 2016, dolichosigma and bowel obturation were diagnosed in patient, and laparotomy and sigma resection with end-to-end anastomosis were performed.
