Topical clonazepam in the treatment of burning mouth syndrome (BMS) – preliminary report.

Abstract
Introduction. Burning mouth syndrome is a chronic oral pain condition without any signs of mucosal pathology, described most often as burning of the tongue. The drugs used in the therapy of BMS belong to different pharmacological groups: antidepressants, antiepileptic drugs, analgesics and mucosal protectors. Topical clonazepam showed the best efficacy in treatment of BMS. Aim of the study. To evaluate the efficacy of the topical use of clonazepam in the group of 30 patients with BMS. Material and methods. 30 patients with BMS underwent an anamnesis, a physical examination and psychological test: State-Trait Anxiety Inventory (STAI). All the patients were instructed to suck a 1 mg clonazepam tablet 3 times a day and then to spit out. The intensity of burning was assessed on the VAS scale: at baseline and after 3, 6 and 12 weeks of treatment. The patients were asked about the side effects of the drug. Results. 20 patients completed the whole 3-month treatment. Very effective treatment was observed in 13 patients (in 5 cases it was total remission), partially effective treatment was observed in 4 cases and treatment failure in 3 patients. The side effects were reported by 13 patients. Among the group examined by psychologist (30 subjects), 56.5% had high scores in trait anxiety, 30% had high degree of state anxiety. Conclusion. Topical clonazepam alleviates oral burning in most cases. It is essential to perform subsequent studies on larger groups of patients with the use of placebo. Psychological factors such as anxiety and stress can be the cause of burning mouth syndrome.
