TREATMENT AND CLINICAL RESULTS OF RECTOVAGINAL FISTULA

Abstract
Aim: Rectovaginal fistula (RVF) is an abnormal epithelial junction between anterior rectum and posterior wall of vagina. Its etiology is surgical trauma, labor, inflammatory bowel disease and cancer. In our study we present clinical features, operation technique and results of 14 rectovaginal fistula patients operated in our hospital. Material and Methods: We retrospectively analyzed clinical and demographic data of 14 patients with RVF operated in our hospital between January 2002 and December 2018. Results: Median age was 47 (24-79). All of the patients have the complaint of stool and gas discharge from vagina. Diagnosis of fistula was done clinically in 10 patients whereas 4 patients needed barium enema. Mucosal flap reconstruction was performed in 10 patients and colostomy was added in 4 patients. The etiology was cancer in 5, surgical trauma in 4, labor in 2 and inflammatory bowel disease in 1 patient. Mean hospital stay was 4.8 days. Mean follow-up was 22 months. Recurrence was observed in 9 patients. Fistula embolisation was performed in 2 patients with recurrent disease. Conclusion: Etiology is one of the main risk factor effecting success in RVF surgery. Treatment of patients with rectovaginal fistula associated with cancer is extremely hard.
