Utility of scrape cytology in early diagnosis of premalignant and malignant oral lesions - A study of 100 cases at a tertiary care centre

Abstract

Background: The oral squamous cell carcinoma [OSCC] is the most common cancer worldwide and constitutes a major health problem in developing countries, representing the leading cause of death. In India it accounts for almost 40% of all cancers. A significant proportion of OSCC develops from premalignant lesions such as leukoplakia, erythroplakia and oral sub mucous fibrosis (OSMF). The histopathological examination of tissue remains the gold standard, but it is an invasive technique with surgical implication. Therefore, the search for the development of newer diagnostic methods were going on, and it led to the discovery of newer molecular methods as well as exfoliative cytology. Molecular methods are costly enough but Exfoliative cytology of oral cavity is a safe, simple, non-invasive and inexpensive method for the diagnosis of premalignant and malignant lesions of oral cavity. Aims/ objectives: For early detection of premalignant and malignant oral lesions, as well as screening of high risk groups. Material and methods:100 cases were included with suspected lesions, high risk groups and healthy subjects as a control. Oral scrape from these patients were collected by brush, smears prepared on slide, fixed with alcohol and stained by PAP and HandE. Results: Out of 100 patients; 24 cases were of healthy individuals [as control], 20 cases were of inflammatory lesions,47 cases were of premalignant lesion and 9 cases were of malignancy (OSCC). Conclusion: Oral scrape cytology is a rapid, painless, safe, inexpensive method for screening and diagnosing a large number of patients in a short time and can be used in camps also for mass screening of populations.
