Video-assisted transhiatal esophagectomy in the treatment of esophageal and cardia adenocarcinomas

Abstract
Introduction. Poor outcome and low survival rate of patients suffering from adenocarcinoma of the esophagus and cardia both lead for new treatment techniques development. Transhiatal video-assisted esophagectomy (TVE) is one of them.Material and methods. Retrospective analisys of 87 patients treated with TVE between 1994 and 2010 year was performed. The study group included: 72 (82.76%) males and 15 (17.24%) females. Average age of patients was 60.5 ± 8.9 (40-80).Results. Mean operative time was 5.08 ± 1.21 h. The average size of tumor was 4.5 ± 2.04 cm. Esophagus substitute was performed mainly from the stomach 73.56% (n = 48). Most frequent substitute position was posterior mediastinum 78.16% (n = 68). Hospital stay after following surgery was 22.14 ± 19.03 (12-87) days. Postoperative complications occurred in 51.72% (n = 45) patients. Relaparotomy was required in 16.09% (n = 14). Mortality rate was 10.34% (n = 9). Average hospital stay was 33.93 ± 22.7 days. Conclusions. Transhiatal video-assisted esophagectomy is a useful method of surgical treatment of adenocarcinoma of the esophagus.
