A rare complication of Behçet's disease in Turkey: Intestinal perforation

Abstract

Although up to 50-60% of patients with Behcet's disease present with intestinal involvement in Japan and Korea, it is very rare and accounts for only 1.4% of the cases in Turkey. The frequency of intestinal perforation which is an important complication of the disease is yet unknown. Here we present a 33 year-old man who presented to our hospital with a high fever, nausea, vomiting, abdominal pain and massive rectal bleeding following a recent hemorrhoidectomy operation and underwent a right hemicolectomy upon diagnosis of caecum perforation. His clinical history revealed a 12-year history of Behçet's disease which was not properly treated and followed-up despite recurring mucocutaneous lesions. Superficial and deep penetrating ulcers were noted in the resection specimen of the case, along with venule predominant small vessel vasculitis, characterized by neutrophilic phlebitis and lymphocytic venulitis in particular. Intestinal Behçet's disease, is an serious entity, for which the differential diagnosis can be very difficult from inflammatory bowel disease such as Crohn's disease. It can also cause important morbidity with unpredictable intestinal perforations. The case was presented to increase awareness of the issue.
