Aile Hekimliği Uzmanlarının Devlet Hizmeti Yükümlülüğü Kuralarının Değerlendirilmesi

Abstract

Aim: This study aimed to evaluate the family medicine specialists’ compulsory health service (CHS) draw. Methods: This descriptive and cross sectional study was performed by analysing the data of family medicine specialists (FMS) who applied for CHS draw indexed on the official website of the Ministry of Health General Directorate of Management Service between September 2005 and April 2017. Results: A total of 2021 FMS was recruited in the study and 867 (42.9%) of them were male while 1154 (57.1%) were female. The mean number of FMS who applied for CHS draw is 155.46 ± 25.71 annually. 607 (30.1%) persons at 46 faculties of medicine and 1400 (69.5%) persons at 31 Training and Research Hospitals participated in CHS draw after completing their specialist training. The first three cities with the highest number of specialists were trained in İstanbul, Ankara and İzmir respectively. 70% of FMS were assigned to the state hospitals due to the result of the drawings. More FMSs were assigned to places in sub-service service groups than to places in upper health service groups with CHS draw. The most common cities of assignment were Erzurum, Sivas and Erzincan, respectively. Conclusion: It is rather difficult to say that the annual number of FMS graduates is sufficient for the field. Although the FMS are primary care physicians, they complete their training in places which are tertiary level and in upper service group. Approximately 70% of the places to which they are assigned are the state hospitals which are usually in the regions within the sub-service groups. Solving this paradox may be possible if the training is done in all service areas belonging to the primary care system and the primary healthcare services should be the place where the assignments are made.
