Anti-HMGCR Antibody Associated Necrotizing Autoimmune Myopathy-A Case-Based Clinical Review

Abstract
A 54 years gentleman presented with proximal and distal myopathies with a history of statin exposure with persistent symptoms posts statin cessation. CPK ranged from 18000 to 25000 IU/L. Lower limb MRI showed extensive bilateral muscle oedema consistent with myositis. EMG was consistent with an inflammatory myopathic disease. Muscle histopathology revealed evidence of fibers' degeneration and necrosis in the absence of inflammation. Improvement in clinical symptomatology and muscle enzymes showed with pulse steroids and other immunomodulators including Methotrexate, IVIG and Rituximab.
