ANTIBIOTIC UTILIZATION IN ACUTE EXACERBATION OF ASTHMA AND COPD

Abstract
One of the most common reasons why patients are presented to the emergency departments or primary care physicians is due to exacerbation of their asthma or COPD symptoms. Excessive use of antibiotics has been a growing concern as it increases the antimicrobial resistance. Despite several guidelines on the management of exacerbations of asthma and COPD, the value of antibiotic in it remains extremely controversial. In this before-after retrospective study, 104 asthma and chronic obstructive pulmonary disease patients attending the out-patient department of Pulmonary Medicine of a medical college hospital in Hyderabad, India were recruited for the study. At two different visits patient’s prescriptions were assessed and data was collected. Comparisons were made between the two visits for the antibiotics prescribed. The results revealed that azithromycin was the most prescribed drug in both first (25%) and second (4.8%) visits, no antibiotic were prescribed to 42 subjects in the first visit whereas this number increases to 94 in the second visit. Amoxicillin+calvulonic acid, Cefepime+tazobactam, Cefpodoxime, cefuroxime, fluconazole and roxithromycin were prescribed in the first visit but not in the subsequent visit. The findings of this study reveal that, antibiotic prescription in exacerbations of asthma and COPD was due to underlying infection. The respiratory condition of the subjects was improved upon prescribed with an antibiotic for 3-5 days. The highest used antibiotic class was macrolide.
