Are silicone stents useful in the treatment of benign esophageal stenoses? Experience of four cases

Abstract

Objective. The management of recurrent benign esophageal stenoses is a difficult problem in general thoracic practice, especially in patients who are poor surgical candidates or strongly reject surgical procedures. We evaluated the role and efficacy of silicone endoprostheses in four patients who had strictures that were resistant to repeated balloon dilation. Patients and methods. We treated four patients using silicone esophageal endoprosthesis as a stent. Each stent had been in place at least for one year. We had to replace the originally inserted stent in two patients annually due to changes of the silicone. Results. Placement of esophageal silicone stent reestablished swallowing in all patients. All patients were swallowing normally on long-term follow-up. No major complications (bleeding, tracheoesophageal fistula, migration into the stomach) occurred on follow-up. Conclusions. Our preliminary experience indicates that silicone endoprostheses can be effective in the management of a selected group of patients who have benign esophageal stenoses and in whom repeated dilations have failed. We think that silicone endoprostheses are more suitable in those selected cases comparing with metallic, plastic covered and uncovered stents.
