Changes in HIV prevalence and intervention service utilization among MSM under sentinel monitoring in Taizhou of Zhejiang Province

Abstract
ObjectiveTo analyze the changes in HIVï¼ syphilis and hepatitis C prevalence among men who have sex with men ï¼MSMï¼ in Taizhou Cityï¼ Zhejiang Province from 2017 to 2022ï¼ and the utilization of HIV intervention services and its influencing factors.MethodsMSM participants were recruited from HIV sentinel surveillance in Taizhou through snowball sampling method. Participantsâ socio-demographic characteristicsï¼ sexual behavior characteristicsï¼ knowledge of HIV and utilization of HIV intervention services were collected and statistically described by questionnaire survey. Blood samples from MSM sentinel population were also collected for HIVï¼ syphilisï¼ and HCV antibody testing. Multivariate logistic regression analysis was used to analyze the influencing factors for utilizing HIV intervention services among MSM.ResultsA total of 2 499 MSM were included in the analysis from 2017 to 2022ï¼ with HIV prevalence of 5.8%ï¼ syphilis prevalence of 4.0%ï¼ and HCV prevalence of 0.6%. The infection rateï¼ proportion of homosexual anal sex and heterosexual sex showed a decreasing trendï¼ while the proportion of condom use during homosexual anal sexï¼ condom publicity and distributionï¼ AIDS consultation and testingï¼ acceptance of peer educationï¼ and online temporary homosexual sex showed an increasing trend. All these differences were statistically significant. Multivariate logistic regression analysis showed that COVID-19 epidemic ï¼aOR=1.66ï¼ 95%CIï¼1.04â2.64ï¼ and HIV knowledge awareness ï¼aOR=2.57ï¼ 95%CIï¼1.22â5.40ï¼ were positively correlated with increased acceptance of condom publicity and distributionï¼ and utilization of AIDS counseling and testing. Those who sought male sexual partners through offline route ï¼aOR=3.44ï¼ 95%CIï¼ 1.16â10.21ï¼ï¼ and had a history of sexually transmitted diseases within one year ï¼aOR=5.47ï¼ 95%CIï¼ 1.04â28.76ï¼ were positively correlated with community-based methadone maintenance therapyï¼ clean needle provision and exchange servicesï¼ while COVID-19 epidemic ï¼aOR=0.35ï¼ 95%CIï¼ 0.14â0.88ï¼ was negatively correlated with the utilization of these services. COVID-19 epidemic ï¼aOR=2.11ï¼ 95%CIï¼ 1.54â2.89ï¼ï¼ HIV knowledge awareness ï¼aOR=2.09ï¼ 95%CIï¼ 1.08â4.02ï¼ï¼ offline search for male sexual partners ï¼aOR=1.47ï¼ 95%CIï¼ 1.03â2.11ï¼ï¼ and awareness of HIV infection of sexual partners ï¼aOR=2.09ï¼ 95%CIï¼ 1.62â2.69ï¼ were positively correlated with the increased use of peer education serviceï¼ while married or cohabited ï¼aOR=0.73ï¼ 95%CIï¼ 0.55â0.98ï¼ and VCT ï¼aOR=0.61ï¼ 95%CIï¼ 0.46â0.82ï¼ were negatively correlated with utilization of peer education service.ConclusionHIV infection rate of sentinel MSM population in Taizhou decreasesï¼ while the utilization of HIV intervention services increases in general. Targeted measures should be developed according to the changes in sexual behavior and HIV intervention services among MSM populations in Taizhou to improve the HIV intervention services for the MSM population.
