Corticotomy - a procedure to speed-up orthodontic treatment

Abstract
Orthodontic treatment is now becoming more and morepopular, especially among adult patients. Development ofknowledge have broadened the range of treatment optionsfor adult patients, who often have specific requirementsconcerning aesthetics, the type of orthodontic appliance,and, in particular, the duration of treatment. The responseto the above-mentioned requirements is provided by theapplication of corticotomy procedure, which enables theshortening of orthodontic treatment time, facilitatesexpansion of the dental arch, accelerates the process ofbringing retained teeth to the arch, and also improves thestability of orthodontic treatment results. Aim. The aim ofthis study was to review the current literature on corticotomyprocedure with special consideration of studies related tothe application of corticotomy for the purpose of toothmovement acceleration and reduction of orthodontictreatment duration. Material and methods. A review of theliterature from 1959-2014 has been conducted. The PubMedmedical database has been searched using the followingkey words: corticotomy, acceleration of orthodontic treatment,and decortication. Thirty-three studies most pertinent forthe subject of this paper were chosen. Results andconclusions. Corticotomy is a procedure consisting inselective decortication of the alveolar ridge by makingdecortication cuts in the tooth movement area. Its aim is toobtain faster bone remodeling in the surgery site and,consequently, to accelerate tooth movement in this area.With the use of the corticotomy procedure, orthodontictreatment time may be reduced to one third that conventionaltreatment time. Presently, there are more and moreindications for corticotomy in orthodontic treatment; amongothers, they include treatment of medium and high intensitydental crowding shortening the therapy duration , supportingthe eruption of impacted teeth as well as ensuring greaterstability of orthodontic treatment effects.
