Defibrillators: historical overview and current status

Abstract
As is known, the main cause of most deaths is cardiac arrest. The article is devoted to topical issues of restoration of the heart, defibrillation and construction of equipment for its implementation. The work of the heart in the processes of fibrillation and defibrillation is described. A thorough review of the historical aspects of the development of defibrillation equipment in Ukraine and abroad is performed. The contribution of the Lviv plant REMA in the development of technology of defibrillation is overviewed. A thorough classification of defibrillators is proposed and the features of functioning and application of each type are described. The questions of influence on the parameters of the discharge pulse and pulse shape, amplitude and duration, type and parameters of the defibrillation voltage are considered. The questions of selection of parameters of the discharge pulse for maximum efficiency of defibrillation are considered. For this purpose, the main forms of discharge pulse, defibrillation schemes and the main energy doses of defibrillation depending on the age of patients are described. The novelties in the technique of defibrillation are described; it is a possibility of application of telemedicine technologies in modern defibrillators for removal of data on cardiac activity at a distance to a doctor; features of the use of subcutaneous defibrillators, their difference from the classic implant and the possibility of using unmanned aerial vehicles for delivery of automatic defibrillators to remote and inaccessible places for emergency care even before the arrival of ambulance. The task for further research is set.
