Difficult airway in a patient with Wegener's Granulomatosis - a case report

Abstract

Wegener's Granulomatosis is an immunologically mediated multi-system disease of unknown etiology. It is characterized by the triad of necrotizing granulomas of the upper and lower respiratory tract, focal necrotizing glomerulonephritis and systemic vasculitis of small arteries and veins. Patients of Wegener's granulomatosis are a challenge to anaesthesiologist due to multisystem involvement of upper airways and various body systems. These patients require proper systematic approach in its anaesthetic management. We are reporting a patient of 50 year old male, having Wegener's granulomatosis, posted for nasal septum biopsy and debridement of lesion. The patient had difficult mask ventilation, difficult laryngoscopy, difficult intubation and difficult extubation. The patient was managed with utmost care and surgery was conducted successfully without any morbidity.
