Radial forearm microvascular free flap for reconstruction of mucosal cheek defect

Abstract

After large oncologic resections, reconstruction of resulting defects is best managed by microvascular free flaps as they are highly successful and relatively safe method. The Radial Forearm Free Flap (RFFF) provides wellvascularized and relatively thin and pliable soft tissue. Amiddle aged male patient with Verrucous carcinoma of left buccal mucosa cheek, was operated using wide local excision and reconstructed using Radial Forearm Free flap. The flap was harvested pedicled on Radial artery together with associated venae commitans and cephalic vein. The microvascular anastomosis of Radial artery with Facial artery and one branch of Venae commitans with Common facial vein was done using surgical microscope to vascularise the flap. The donor site was covered with a Full Thickness Skin Graft (FTSG) harvested from groin area. The flap was taken up very well with minimal morbidity at donor site. RFFF has high success rate, provides good aesthetic and functional outcome, which allows reconstruction of various intraoral defects.
