Endoscopic removal of sphenoethmoid cemento-ossifying fibroma

Abstract
Introduction: Cemento-ossifying fibromas (COF) are rarely encountered in ENT practice. They are even rarer in spheno-ethmoid region. Endoscopic excision of COF has only once been reported in literature. We report a unique case of endoscopic excision of cemento-ossifying fibroma of spheno-ethmoid region. Case Report:Aforty year male was referred to E.N.T outpatient department from ophthalmology, for complaints of bilateral decreased vision of 3 months duration which was insidious in onset, painless and progressive. The patient had associated headache which was present in the vertex and occipital area. ENT examination was essentially normal including diagnostic nasal endoscopy. On ocular examination the best corrected visual acuity in the right eye was 6/12 and left eye was 6/36. CT scan nose, paranasal sinuses and orbit revealed a well-defined oval hyper dense lesion arising from the antero-lateral aspect of the right sphenoid sinus involving posterior ethmoids. The CT scan findings were corroborated by MRI. Endoscopic excision of the mass was done and patient was free from headache in the follow up. Conclusion: Endoscopic excision of COF should be considered whenever possible as this reduces the morbidity and hospital stay of the patient.
