Evaluation of changes and diagnostic informative of the cytokine profile in young patients with gastroesophageal reflux disease, occurring against the background of autoimmune inflammation

Abstract
Objective — to determine the general trend of changes in the indicators of systemic inflammation, interleukin 1β (IL 1β) and interleukin 18 (IL 18) in patients with gastroesophageal reflux disease and autoimmune thyroiditis in young people. Materials and methods. The study of the role and levels of IL 1β and IL 18 involved 83 young patients (students from 18 to 25 years) with gastroesophageal reflux disease (GERD) and autoimmune thyroiditis (AIT). The comparison group consisted of 30 patients with isolated GERD of the matched age, gender, and form of the disease. Results and discussion. It has shown that the GERD was accompanied by a significant increase of IL 1β levels, in association with AIT this indicator increased in 1.7 times. It was established that the value of IL 1β depended on the form of the esophageal mucosa lesions, it was more pronounced in the presence of erosion, but did not correlate with their number. In patients with GERD and AIT, the IL 18 levels were increased up to 1761.5 (1451.7; 2876.9) pg/ml, which exceeded this values for patients with isolated GERD: 614.6 (521.9; 721.8) pg/ml and control values (p < 0.05). Conclusions. The increased IL 18 levels have been reveled in patients with GERD and AIT, due to the inflammatory process in both organs and an autoimmune process in the thyroid gland. Further disease progression may occur due to the preservation of the autoimmune component. The GERD course, especially at it combination with AIT in young people, was accompanied by increased IL 1β levels in peripheral blood, thus indicating the presence of an active inflammatory process.
