Investigation of an outbreak of Mycoplasma pneumoniae respiratory tract infection in a primary school in Huzhou Cityï¼ Zhejiang Province

Abstract
ObjectiveTo understand the epidemiological characteristics of an outbreak of respiratory tract infections caused by Mycoplasma pneumoniae ï¼MPï¼ in a primary school in Huzhou Cityï¼ Zhejiang Provinceï¼ and to provide scientific reference for future prevention and control of school outbreaks of upper respiratory tract infections caused by M. pneumoniae.MethodsAccording to the requirements for investigating respiratory infectious disease outbreaksï¼ basic informationï¼ clinical history and epidemiological information were collected from the cases. Throat swab samples of cases for COVID-19 nucleic acid testingï¼ influenza A and B virus antigen testingï¼ and serum samples for detecting total antibody of mycoplasma were also collected.ResultsA total of 47 cases related to M. pneumoniae infection were identified. The first case occurred on May 4ï¼ 2022ï¼ and the cases were concentrated between May 22 and May 31ï¼ 2022. The last case occurred on June 21ï¼ marking a duration of 48 days for the outbreak. All cases were second-grade studentsï¼ with an incidence rate of 35.07% ï¼47/134ï¼ in the affected classesï¼ with Class 206 at 53.330% ï¼24/45ï¼ï¼ Class 207 at 38.64% ï¼17/44ï¼ï¼ and Class 211 at 13.33% ï¼6/45ï¼. There was a statistically significant difference in the incidence rates among the classes ï¼Ï2=16.57ï¼P<0.05ï¼. The incidence rate was 32.39% ï¼23/71ï¼ for boys and 38.10% ï¼24/63ï¼ for girlsï¼ with no statistically significant difference by gender ï¼Ï2=0.48ï¼P>0.05ï¼. The main clinical manifestations included coughï¼ feverï¼ and sore throatï¼ and some cases exhibited signs of pneumonia. Pharyngeal swabs and serum samples were collected from 27 patients. The results were negative for pharynx swab samplesï¼ and positive for total mycoplasma antibody in 12 serum samplesï¼ with a positive rate of 44.44%.ConclusionSymptoms caused by M. pneumoniae are diverse in clinical manifestationsï¼ with atypical signs and a slow progression. Early cases tend to exhibit mild symptoms. The onset of the disease is not detected in time when students are attending classes. The school morning health checks fail to promptly detect it and trigger home isolation control measuresï¼ which all contribute to the outbreak of the epidemic. Schools should conduct relevant prevention and control campaignsï¼ improve morning and afternoon health checksï¼ and prevent students from attending classes while sick.
