Leczenie bólu w chorobach reumatycznych

Abstract
Przewlekły ból jest kluczowym objawem większości chorób reumatycznych. Ponieważ na choroby układu kostno-stawowego cierpi ok. 70% populacji po 50. roku życia, są one najczęstszą przyczyną przewlekłych zespołów bólowych. O bólu przewlekłym mówimy, gdy trwa powyżej trzech miesięcy. Zrozumienie podstawowych mechanizmów powstawania i transmisji bodźców bólowych pozwala wdrażać skuteczne metody postępowania leczniczego. W każdym przypadku bólu przewlekłego niezbędne jest ustalenie jego przyczyny i ocena wydolności nerek i wątroby. Na czoło tych metod wysuwa się farmakoterapia z coraz powszechniejszym zastosowaniem leków z grupy silnych opioidów, takich jak: pochodne morfiny, fentanyl, buprenorfina, które wraz z niesteroidowymi lekami przeciwzapalnymi i paracetamolem stanowią grupę leków pierwszego rzutu. W artykule omówiono najważniejsze zagadnienia z zakresu farmakoterapii bólu.
