REBT WITH PHARMACOTHERAPY IN THE MANAGEMENT OF CHRONIC PAIN- A QUASIEXPERIMENTAL STUDY

Abstract
BACKGROUND In the recent past, there has been an increase in the number of patients who seek specialist neurological consultations for chronic pain which need longterm pharmacotherapy due to associated biopsychosocial issues. The aim of the present study was to evaluate the efficacy of Rational Emotive Behaviour Therapy (REBT) and Pharmacotherapy in the management of pain. The objectives of the present study were to determine the associated underlying anxiety, depression and stress in patients with chronic pain; and to compare the efficacy of different treatment modalities like REBT, psychoeducation and pharmacotherapy in the management of chronic pain. MATERIALS AND METHODS 150 patients with chronic musculoskeletal pain and primary headache attending the Neurology Outpatient Clinic of a teaching hospital were enrolled through convenience sampling. They were divided into three groups, with 50 patients in each group. Allocation of samples to different groups was done by simple randomisation method. They received the following treatments: (i) Pharmacotherapy with REBT, (ii) Pharmacotherapy with psychoeducation and (iii) regular pharmacotherapy. Numerical pain rating scale (NRS) and Depression, Anxiety and Stress Scale (DASS) were used to assess the patients at entry and at the end of 3 months of treatment to evaluate the severity of pain and psychological comorbidities like depression, anxiety and stress respectively. Settings and Design- A quasi-experimental intervention study was conducted in a tertiary care hospital in south India. Statistical Analysis- Chi square test, âtâ test and Kruskal-Wallis hypothesis test were used to prove the statistical significance. RESULTS A statistically significant, beneficial effect was observed in the Numerical pain rating scale and DASS scores in the group of patients who received pharmacotherapy in combination with Rational Emotive Behaviour Therapy. CONCLUSION We recommend REBT, a simpler form of psychotherapy to be effective in combination with pharmacotherapy in the management of chronic pain.
