Study of laparoscopic cholecystectomy and its conversion to open cholecystectomy: Analysis of 42 cases

Abstract
Background: Today, laparoscopic cholecystectomy (LC) has revolutionized minimally invasive surgery and become the gold standard for symptomatic gallstones. Laparoscopic cholecystectomy has now become a standard option, but may also be an operative approach with underlying complexities necessitating conversion to open cholecystectomy. We have tried to estimate the rate and risk factors which are associated with the conversion. Laparoscopic cholecystectomy offers a cure for gallstones with a minimally invasive procedure, minor pain and scarring, and early return to full activity. Methods: This study analyzes the conversion rate of laparoscopic cholecystectomy to open cholecystectomy in Department of General Surgery, Yenepoya medical college and hospital Mangalore. This is a retrospective study of 42 patients conducted from July to September 2018. Also the purpose of our study is grading of difficult or problematic Laparoscopic cholecystectomy and studying the outcome Laparoscopic cholecystectomy cases, its complication and management and to decide when to convert Laparoscopic cholecystectomy to open cholecystectomy. Results: Out of the 42 cases 39 were successfully operated by Laparoscopic cholecystectomy. Only 3 cases out of 42 were converted from Laparoscopic to open cholecystectomy. Conclusions: Laparoscopic cholecystectomy is a safe method of treatment with very low conversion rate. Laparoscopic cholecystectomy intra operatively were graded, 1 to 5 where Grade 1 is very easy level of performing Gall bladder surgery to Grade 5 where expected complications and conversion rate to open is high.
