Subphrenic abscess. Late complication of laparoscopic cholecystectomy. Case report

Abstract

Numerous advantages of laparoscopic cholecystectomy are the reason why this method is a gold standard for uncomplicated cholelithiasis. In addition to common bile duct injury, this method is also correlated with a higher risk of gallstone spillage. Unretrived gallstones are not regarded as a dangerous complication by some surgeons, however serious complications can develop as an effect of gallstones unretrived from the peritoneal cavity. A case of patient who was diagnosed because of general weakness and fever is presented. A subphrenic abscess was revealed in additional examinations. A gallstone was found during the subsequent operation. This complication developed four years after laparoscopic cholecystectomy. Gallstone spillage during laparoscopic cholecystectomy is a rare cause of complications, however serious complications can develop after gallstones spillage sometimes. There is no algorithm specifying what should be done in such situations. A careful preparation and insertion of gallbladder into a special pouch before evacuation could eliminate gallstone spillage.
