Survey of Hospital Errors in Teaching Treatment Centers in Kermanshah Province

Abstract

Reviewing and classifying most important types of errors may help eliminate underlying causes of errors. The present study aimed to examine hospital errors in teaching treatment centers in Kermanshah province. This is an applicable descriptive - analytical study conducted as a survey Error report form of Ministry of Health and Medical Education was used to collect data. This form was presented to all health centers from beginning of 2011. It was collected on monthly bases. The statistical population included all medical staff of health centers in Kermanshah Province among whom those who attempted to report the errors were selected as sample. The sample size was equal to number of errors reported from health centers in 2011. Data was analyzed using descriptive statistics with SPSS 18. The findings showed that distribution of frequency of errors with the highest number of errors was reported on A hospital with 395 cases (56.4%) while the lowest one was reported on B hospital with 24 cases (3.4%). Distribution of the highest number of errors in terms of error types called systemic errors included 213 cases (30.4%) while the lowest number of errors called the wrong path included 6 cases (0.9%). Moreover, percentage of errors increases per year. The difference was statistically significant during three years under study (P = 0.01).managers and administrators of the center should review policies regarding error reporting and organize culture of non-punitive reporting system to encourage the staff to report errors. Then, the managers would deal with the errors using a system-based approach.
