The imaging of sterno-clavicular joints in SAPHO syndrome

Abstract
SAPHO syndrome is seronegative spondyloarthropathy characterized by dermatological symptoms such as pulstulosis palmo-plantaris and acne, and osteoarticular symptoms.<br>Diagnostic procedures of SAPHO syndrome include radiographic assessment although the presence of radiologic changes does not belong to diagnostic criteria.<br><b>Material and methods. </b>Fifteen patients (14 female, 1 male) with confirmed diagnosis of SAPHO syndrome were studied. All patients underwent bone scintigraphy, routine x-ray and computed tomography of sterno-clavicular joints.<br><b>Results.</b> Increased tracer uptake on bone scintigraphy was found in 13 (87%) of patients. In 9 (60%) of them in sterno-clavicular joints, in 4 (27%) – in sacroiliac joints and in 4 (27%) – in peripheral joints. In routine x-ray assessment of sternoclavicular joints abnormalities were found in 4 (27%) of patients, including: clavicle enlargement, erosions with hyperostosis, irregular calcifications, irregular surface of clavicles. On CT assessment of sterno-clavicular joints abnormalities were found in 6 (40%), including geodes, hyperostosis and erosions. All patients with abnormal result of CT had abnormal bone scintigraphy of sterno-clavicular joints. Four of them had abnormal result of routine x-ray. Two of them had normal result of routine x-ray.<br><b>Conclusions.</b> In patients with SAPHO syndrome routine x-ray in not sensitive and seems not necessary in diagnostic procedures. CT assessment is more sensitive in this group of patients. Bone scintigraphy is very sensitive but it could be nonspecific.
