The incidence of white spot demineralization in patients treated with fixed orthodontic appliance

Abstract
White spot demineralization (or white spot lesions - WSLs) in patients with inadequate oral hygiene is one of the most common adverse phenomena that occur during orthodontic treatment with fixed thin archwire appliances. A fixed appliance represents an element disturbing the natural mouth ontocenosis since, on the one hand, it curbs the flow of saliva and the possibility of natural tooth cleaning, and, on the other hand, it is a place of retention of food remnants and bacteria, contributing to the development of white spot lesions. [b]Aim.[/b] To determine the incidence in archwire appliances. [b]Material and methods. [/b]The study group consisted of 100 randomly selected patients treated with fixed thin archwire appliances. The patients were divided into 3 groups depending on the duration of orthodontic treatment. The incidence, number, and location of white spot lesions on the labial and buccal surfaces of tooth crowns were visually checked. The severity of demineralization was assessed using the Gorelick index. The oral hygiene was evaluated with the PIOP index. [b]Results.[/b] White spot demineralization was observed in 20 to 46.67% of the patients. The longer the duration of the treatment, the higher the incidence of white spot lesions was. The incisors and canines were the were most frequently affected with white spot lesions. Demineralization was more common in males than in females. The worse the oral hygiene status, the higher the incidence of white spot lesions was reported. [b]Conclusions[/b]. The study revealed a correlation between the oral hygiene status and the occurrence of white spot lesions on tooth enamel surface.
