The mebicar efficacy in correction of the psycho-emotional background in patients with stable ischemic heart disease with permanent atrial fibrillation

Abstract

Objective — to assess the dynamics of the psycho-emotional background under the influence of adminitrsation of the anxiolytic drug mebicar. Materials and methods. The observations were performed in 31 patients with stable coronary heart disease (SCHD) in combination with atrial fibrillation (AF). The patients were divided into 2 groups: 1a — patients with SCHD and permanent AF who received standard therapy (16 patients were examined), 1b — patients with SCHD and permanent AF receiving anxiolytic therapy (15 patients were examined). To assess the susceptibility of chronic stress, the L. Reeder questionnaire and perceived stress 10 were used, and the hospital scale HADS and PHQ 9 were used to analyze the degree of anxiety and depression. The state of health was evaluated with the use of PHQ 15 questionnaire. The analysis of the risk factors for cardiovascular disease was carried out according to the questionnaire O.S. Chaban. Results and discussion. The results showed low stress level only in 37.50 % of patients in group 1a, while in group 1b it was 6.67 % (p < 0.01). The positive dynamics of treatment with anxiolytic drug was defined in men of 1b group (p < 0.05). The high stress in men with SCHD and AF (26.67 %) was established more frequently than in women (13.33 %) (p < 0.01). Overall, 46.67 % of men had clinically expressed anxiety-depressive symptoms. (p < 0.01). The probable difference in the rates of depression in group 1b after treatment has been demonstrated (p < 0.01). A similar pattern was detected for the severity of depression in 33.33 % — respectively (p < 0.05). The tendency to subclinically expressed anxiety (p > 0.05) was established in women with SCHD and AF in 1a and 1b groups. The role of social factors has been assessed, including reducing of the time of concurrent conduct, the influence of the media in the respective groups of surveyed (p < 0.05). The deterioration of the mutual understanding in both groups of patients (p < 0.01) after the treatment was observed. Conclusions. Thus, the assessment of the psychosomatic status of patients with SCHD in combination with AF is one of the important directions in modern medicine, since timely detection and correction of clinically expressed stress, anxiety and depression can improve the quality and life expectancy of people with major illness. Therefore, the mebicar inclusion in the basic pharmacotherapy of patients with SCHD in combination with AF can be relevant and promising in cardiology.
