The median arcuate ligament syndrome as a rare cause of motor activity disturbances of the stomach

Abstract

The median arcuate ligament syndrome is a rare disease and clinically it is characterized by occurrence of vomiting, nausea and abdominal pain, attributed to compression of the celiac axis by the median arcuate ligament. The pathophysiologic origin of these symptoms is not clearly understood, there are considered neurogenic or vascular origins, but none of them was proved. We show the case of fourteen-year-old boy, who was admitted to hospital because of: abdominal pains localized in the epigastrium, vomiting, nausea and lost of weight.The endoscopy of the upper part of the alimentary tract showed extensive inflammatory changes of the mucous membrane of the esophagus, the stomach and deformation of the pylorus. The radiological examination of the upper part of the alimentary tract showed a very large stomach, the lower part extended to the iliac ala, however the stomach emptying was normal.The electrogastrography and scintigraphy showed considerable functional disturbances in the upper part of the alimentary tract. On the ground of Doppler examination of the abdomen (difference of Doppler spectrum during the inspiration and expiration within the celiac axis) and angiocomputer tomography we diagnosed the median arcuate ligament syndrome in this patient. We observed regression of symptoms in the boy after included treatment: pharmacological and diet. Control endoscopy was normal, too. At the moment the patient is under observation in Gastroenterological and Surgical Clinic, with the aim of qualification to operative treatment. In the presented report we want to pay attention to the rare cause of functional disturbances of the alimentary tract, as it is the median arcuate ligament syndrome and the immense role of Doppler examination of the abdomen in assignation of the final diagnosis.
