The significance of electron microscopy in elucidation of pathogenesis of acute pancreatitis

Abstract
Acute pancreatitis is generally believed to be a disease in which the pancreas is injured by digestive enzymes that it normally produces. The mechanisms responsible for intracellular activation of digestive enzyme zymogens have not been elucidated with certainty. This paper reviews the ultrastructural changes in acinar pancreatic cells which have been the most significant in elucidation of pathogenesis of acute pancreatitis. This review focuses on the ultrastructural evidences supporting the validity of the main pathogenetic hypotheses: co-localization and autophagy. The paper emphasizes the role of ultrastructural studies in the connection of pancreatic microcirculation disturbances with severity of disease and the importance of oxygen radicals in the pathogenesis of acute pancreatitis.
