Treatment modalities, complications and prognostic factors of Thyroid carcinoma

Abstract
Background: There is increase in the relative risk of about 45% for malignant tumors and 15% for benign tumors in children exposed to therapeutic low dose radiation. A 3-5 year latency period was noted between the radiation exposure and tumor development. About 90% of radiation exposed cancers were found to be papillary thyroid carcinomas. Methodology: The patients admitted to District hospital attached to Medical College with diagnosis of carcinoma thyroid were taken for this prospective study. The patients selected for this study are those who are admitted with clinically palpable thyroid swelling. Based on detailed history, thorough clinical examination and relevant investigations like FNAC, USG of thyroid and others, the diagnosis of carcinoma thyroid was made. Results: Most common presenting symptom was swelling in the neck. Duration of the swelling varied from case to case with the longest being 15 years and shortest being 15 days. Cervical lymph node suspected of secondaries was present in 12 cases.Eight patients came with occult carcinoma, they presented with cervical lymphnode enlargement only. Two patients were of follicular variant of papillary carcinoma. Two patients were associated with Hashimoto’s thyroiditis. Conclusion: Post-operative complication of wound infection occurred only in 2 cases with an incidence of 5.2%.
