Two-stage diagnostic scheme (enzyme-linked immuno sorbent assay and immunoblot) of Lyme borreliosis among patients with pulmonary tuberculosis

Abstract
Objective — to learn the prevalence of Lyme borreliosis among patients with pulmonary tuberculosis, using a two-stage diagnostic scheme: enzyme-linked immuno sorbent assay (ELISA) and immunoblot. Materials and methods. In the screening study of Lyme-borreliosis, 99 patients with pulmonary tuberculosis were examined, which were treated in-patients conditions of the Ternopil region TB dispensary during 2015—2017. A two-stage diagnostic scheme (ELISA and immunoblot) were used. In the first step, the IgM and/or IgG antibodies to the B. burgdorferi sensu lato complex in the blood serum were determined by the immunoassay (ELISA). According to the results of the first stage of the study, blood specimens with questionable and positive results were re-examined using the immunoblot method. Results and discussion. The presence of antibodies to at least one IgM and/or IgG class in the B. burgdorferi sensu lato complex (B. burgdorferi, B. afzelii and B. garinii) in serum was detected in 25.2 % of patients with pulmonary tuberculosis. Immunoblot testing (EUROLINE RN-AT) confirmed the presence of IgM and/or IgG antibodies against the antigens of B. burgdorferi sensu lato (B. burgdorferi sensu stricto, B. afzelii and B. garinii) (positive or intermediate findings) at 19 (76.0 %) out of 25 respondents with positive and intermediate results in the first phase of the study (ELISA). Conclusions. In a screening study, the prevalence of borrelia infections in patients with pulmonary tuberculosis was 25.2 % in the first stage of diagnosis (ELISA) and 19.2 % in the second (immunoblot), confirming the need of two-stage serologicalsheme of Lyme borreliosis diagnosis.
