ВЛИЯНИЕ ОЖИРЕНИЯ НА КАРДИОГЕМОДИНАМИЧЕСКИЕ И ЭХОКАРДИОГРАФИЧЕСКИЕ ПОКАЗАТЕЛИ У ПАЦИЕНТОВ С ХРОНИЧЕСКОЙ СЕРДЕЧНОЙ НЕДОСТАТОЧНОСТЬЮ И СОХРАНЕННОЙ ФРАКЦИЕЙ ВЫБРОСА ЛЕВОГО ЖЕЛУДОЧКА

Abstract
Objective –to study the effect of obesity on the changes in the indices of ion homeostasis, aldosterone and brain natriuretic peptide, as well as on the parameters of cardiohemodynamics and echocardiography in patients with CHF and LVEF. Materials and methods.The study included 81 patients with CHF II-III FC and LVEF at the age of 39 to 89 years. The results were processed using the statistical software package SPSS 17.0. Statistically significant differences in the data and correlation between them were considered for p ≤0.05 Results and discussion.The increase in body weight in patients with CHF and LVEF was associated with a decrease in the content of aldosterone. There was a trend towards an increase in the level of NTproBNP in the blood serum as compared to the value of this biomarker in individuals with overweight and obesity. With the increase in BMI, there was a deterioration in the quality of life of patients with CHF and LVEF, who were overweight or obese. The 6-minute walk test proved to be higher in patients with normal body weight, but in all patients with CHF and LVEF it was significantly lower than in those of the control group. There was a tendency to worsen diastolic function of the left ventricle with an increase in the patient's body weight, while the lowest values of E / A were found in obese individuals. The LV EF values in patients with CHF and LVEF were somewhat reduced. Conclusions. The increase in body weight in patients with CHF and LVEF is not accompanied by significant changes in the indices of ion homeostasis, aldosterone and NTproBNP in the blood serum of such patients. In patients with normal body weight, there is a tendency to hyponatremia to an increase in the level of aldosterone and NTproBNP in the blood serum as compared to individuals with overweight and obesity. With the increase BMI in patients with CHF and LVEF, there was a tendency to an increase heart rate, systolic and diastolic blood pressure levels, reduced exercise tolerance and quality of life parameters, reaching a reliability criterion in patients with overweight and obesity in comparison with the control group. The increase BMI in the patients under study is accompanied by worsening of echocardiographic parameters. The presence of obesity in patients with CHF and LVEF is associated with a significant deterioration in diastolic function of the left ventricle.
