1,000 operations of carotid artery stenosis in Bielanski hospital – development of opinions inspired by recommendations and own experience

Abstract

Introduction: 20% of brain infracts are caused by carotid artery stenosis. In order to avoid complications, patients are offered a procedure consisting of removal of the sclerotic plaque from common carotid artery bifurcation. A thousand such procedures were performed in the Vascular Surgery and Angiology Unit in the years 2005-2009.Materials and methods: Among the 1,000 patients operated on, there were 68% men and 32% women at the age of 46-92, suffering from numerous concomitant diseases. Carotid artery stenosis was the indication for operation in >70% of symptomatic patients and in >80% patients without vivid neurological symptoms. Patients were qualified for the operation based on Doppler-duplex examination. The operation was performed under the regional anaesthesia.Results: Among the operated patients, 52% had endarterectomy with simple closure of the aterectomy, whereas 34% had enarterectomy with the patch and in 14% patients eversion technique was applied. Carotid artery clamping was applied for10 up to 42 minutes. In 26% patients it was necessary to use internal shunt. Temporary neurological disorders were observed in 7% patients, bleeding from the wound requiring control appeared in 9% patients and 1 % were deceased due to neurological reasons.Conclusions : 1. Operation of sclerotic carotid artery stenosis are an effective and safe method of brain infract prevention. 2. Due to the preventive character of the operation, it should be performed by experienced surgeons, with the possibility of objective intraoperative monitoring.
