A Study effectiveness of hemodynamic responses to laryngoscopy and endotracheal intubation to intravenous dexmedetomidine with respect to different doses

Abstract

Background: The process of intubation is a noxious stimuli leading to a period of extreme haemodynamic stress and is accompanied with intense sympathetic activity marked by tachycardia and hypertension. Aims and Objectives: To Study effectiveness of hemodynamic responses to laryngoscopy and endotracheal intubation to intravenous dexmedetomidine with respect to different doses. Methodology: After approval from institutional ethical committee this study was carried out in the department of anesthesiology in the patients undergoing laryngoscopy and endotracheal intubation during the one-year period i.e. February 2013 to February 2014 with the written and explained consent they were enrolled into two different group with respect to dexmedetomidine used for the attenuation of hemodynamic parameters ; Group I received IV dexmedetomidine 0.5 µg/kg diluted to 20 ml with normal saline as infusion over 10 min. Group II received IV dexmedetomidine 0.75 µg/kg diluted to 20 ml with normal saline as infusion over 10 min. The hemodynamic responses Reponses were recorded in the form of Heart rate, Systemic, Diastolic and Mean Arterial blood pressure. The statistical analysis done by unpaired t-test calculated by SPSS -19 version. Result: The heart rate was significantly higher in Group I as compared to Group II at 2min (P<0.002), 3min (P<0.001), 4min (P<0.05), at 5min (P<0.05) respectively. The systolic blood pressure was significantly higher at2min (P<0.01) and 3min (P< 0.001). The diastolic blood pressure was comparable with each other in both the groups (P>0.05). The Mean arterial pressure (MAP) was significantly higher at 3min (P<0.01), and 4min (P<0.05) Conclusion: It can be concluded from our study from our study that dexmedetomidine at 0.75 µg/kg dose was found superior to 0.5 µg/kg dose with respect to hemodynamic stability so this dose should be used if it is permissible with respect to sedation.
