Diagnosis and treatment of peripheral T-cell lymphoma (PTCL) including enteropathy associated T-cell lymphoma (EATL)

Abstract

The peripheral T-cell neoplasms are a biologically and clinically heterogeneous group of rare disorders that result from clonal proliferation of mature post-thymic lymphocytes. ALK positive anaplastic large cell lymphoma and indolent MF have a good outcome, however 5-year survival rate for other nodal and extranodal T-cell lymphomas is about 30%. CHOP-like therapy with or without an up-front auto-HSCT remains the standard therapy but still treatment outcomes are unsatisfactory and adoption of a more intensive approaches is a reasonable option in fitter patients. Relapse or refractory disease should be treated with second line therapy and allo-HSCT or novel agents should also be considered. Enterophaty associated T-cell lymphoma is an aggressive form of lymphoma, associated with celiac disease and poor prognosis. Standard CHOP therapy gives poor results and long-term survival is rare. Dose intensification is often attempted but it is yet to be confirmed as beneficial in clinical trials.
