Evaluation of sickling and solubility tests and peripheral blood smear method for screening of sickle cell disease

Abstract

There are various tests like sickling, solubility and peripheral blood film methods for the screening of sickle cell disease but their reliability and ease of applicability has not yet been determined. Hence this study has been carried out with an aim to determine the reliability of sickling, solubility and peripheral blood smear methods for the screening of sickle cell disease. This was laboratory based study which has been carried out at Shri Shankaracharya Institute of Medical and Sciences, Bhilai. In this study 300 blood samples which were received in the laboratory from the patients aged 1years to 12 years were analyzed by using sickling, solubility and peripheral blood smear methods. The gold standard method used to establish final diagnosis was heamoglobin electrophoresis on cellulose acetate membrane. Sickling, solubility test and peripheral blood smear methods has shown the sensitivity 58.82 %, 48.39% and 31.43% respectively. Sickling, solubility test and peripheral blood smear methods has shown the specificity 95.49%, 89.96% and 96.60% respectively. While diagnostic accuracy for Sickling, solubility test and peripheral blood film method was 91.33%, 85.67% and 89% respectively. Turnaround time noted for sickling test was 41 minutes, while solubility test and peripheral blood film method required 25 minutes and 45 minutes respectively. The study came out with the conclusion that sickling and solubility test are easy to perform and interpret the result as compared to peripheral blood smear method but sickling test was more reliable as compared to solubility test and peripheral blood smear method. Hence for screening of sickle cell disease sickling test is recommended and positive results should be confirmed with heamoglobin electrophoresis.
